We provide multi-disciplinary palliative care at the hospice in Blurton, in people’s own homes and in community settings such as nursing homes. We’re happy to accept referrals into our Adult Services from any healthcare professional, including GPs, hospital doctors, community nurses and specialist nurses.

Referral criteria

Our adult palliative care services are available to people aged 18 years or over with a life-limiting illness that is active, progressive and the focus of care is quality of life. Additionally, the patient has specific needs which are unmet and requires specialist palliative care to support with these.

Our Dementia Services offer support to patient and their carers within all stages of the disease. Before completing the referral, please view our complete criteria below.

Self-Referrals

We encourage healthcare professionals to make referrals when appropriate, as this allows us to gather key clinical information from the beginning.

However, we do also have a self-referral route where referrals can be made by the patient, their carer or a loved one.
