We’re here to support you

We want to make it as simple as possible for someone who has a life-limiting illness and needs our help to make a self-referral. Self-referrals can be made by an individual, their family, carer or loved one.

We provide specialist palliative care at the hospice in Blurton, in people’s own homes and in community settings. If you are unsure whether you or your loved one could access our services, you may wish to have a conversation with your GP about whether you could be referred to our services.

Self Referral

Who can make a self-referral?

You can refer yourself as someone needing care or a carer or loved one can make a referral on behalf of someone else. Our adult services are provided for people aged 18 and over.

What services are available for self-referrals?

Referrals can be made to any of our adult services. All referrals are reviewed by a member of our team to ensure the most appropriate support is provided.

What if I need more urgent advice or support?

Please call our 24/7 Advice Line on 01782 344300.

Our advice line is available for healthcare professionals as well as for patients and carers.

Is hospice care only for people with cancer?

No, our hospice care (palliative and end of life care) can benefit people who have been diagnosed with a range of life-limiting illnesses including (but not limited to): cancer, dementia, respiratory conditions such as COPD, motor neurone disease and Parkinson’s.

We can also provide support to the individual’s loved ones.

What happens after I have made my referral?

Here’s what happens next:

  • Once we receive your referral, a clinical member of our team will carefully review the information you’ve shared.
  • We’ll contact you within 7 to 14 days to talk through your request in more detail.
  • If you feel you need support sooner, please don’t hesitate to call 01782 344300 and ask for our Advice Line.
  • During our call, we may ask for a little more information to help us understand your needs and ensure we’re offering the most appropriate support. Please have medical letters and records to hand for this call.
  • Following the call if you meet the criteria we will connect you with the right team in the hospice.

On the self-referral form you ask about consent, what does that mean?

Consent means giving permission for care, treatment, or examination after receiving clear, understandable information about what it involves. Or in this initial stage of support, it is provided so that we can explore yours/the referred individual’s medical information to help us determine if our services are suitable.

For consent to be valid, it must be:

  • Voluntary – given freely, without pressure from others
  • Informed – based on a full understanding of the treatment, including risks, benefits and alternatives
  • Capable – the person must have the mental capacity to make the decision

Where a person lacks mental capacity to make a decision a best interest decision will be made following the Mental Capacity Act 2005 involving relevant Health Care Professionals and people important to that person (NOK, family, informal carer, legal representative).

 

Consent can be given:

  • Verbally – saying “yes” to a treatment
  • In writing – signing a consent form
  • Non-verbally – for example, holding out an arm for a blood test

Patients have the right to change their mind at any time, and their decision must be respected – even if it means refusing treatment.

If someone lacks the capacity to consent, healthcare professionals must act in their best interests, following the Mental Capacity Act 2005.
